The overall health status of older or frail people is frequently complicated. Among such populations, paradoxical situations are often present. The obesity paradox and the cholesterol paradox are frequently discussed paradoxes. There may be a need for new cut-offs in BMI and cholesterol for older frail people. Alternative cut-offs also for anaemia and iron deficiency should be considered.
