Discontinuation of drug treatment in remission does not seem to affect the long-term prognosis of epilepsy. However, there is a two-fold risk of seizures for the first 2 years after stopping AEDs. Twenty percent of patients do not become seizure-free immediately after restarting AED after relapse. The list of potential pitfalls of stopping AEDs is long. Patients with juvenile myoclonic epilepsy, those with prior withdrawal attempts and late remission should not be encouraged to stop AEDs. We need new anti-seizure drugs with antiepileptogenic effects to prevent seizure relapse after withdrawal.
