Recent advances in Doppler ultrasound technology have provided anesthesiologists and surgeons with a powerful tool that can aid intraoperative management of patients at risk of neurological injury. TCD is a useful noninvasive intraoperative monitor of cerebral hemodynamics; its benefits can be demonstrated in many specific instances. However, currently there are still logistic as well as practical limitations. Further refinement of these tools will undoubtedly enhance intraoperative safety of the patient.
