Résumé

La détection de la clonalité lymphoïde B ou T par PCR est devenue indispensable dans de rares cas de lésions lymphoïdes d’interprétation difficile sur le seul plan histologique et immunohistochimique. Le programme européen BIOMED-2 a permis d’importantes avancées à la fois dans l’augmentation de la sensibilité des techniques et la standardisation à un niveau multicentrique impliquant 47 laboratoires européens. Toutefois, il importe de bien connaître les limites de ces techniques afin de les interpréter au mieux dans le cadre du diagnostic d’une lésion lymphoïde. Dans ce cadre, un dialogue concerté entre cliniciens, histopathologistes, hématologistes et molécularistes est crucial pour permettre le diagnostic optimal nécessaire à la prise en charge adaptée du patient.

Summary

Lymphoid clonality detection using PCR is becoming crucial in rare cases of lymphoid lesions difficult to interprete at the histo- and immunohistopathogical level. European Concerted Action “BIOMED-2” developed highly efficient PCR protocols as well as standardized methods and reagents involving 47 european Laboratories. However, it is important that these test results are interpreted with the full knowledge of the limits of these techniques. Therefore, close cooperation between clinician, pathologist, haematologist and molecularist are important to optimize the diagnostic and to further adapt the treatment.
