We report a case of “ulcero-mutilating” variant of carpal tunnel syndrome.

It was an old woman with type 2 diabetes and ulcero-osteolytic lesions on left hand.

Lesions were localized on median nerve territory (second and third phalanges).
- •
Other causes were excluded by laboratory, neurologic and radiologic exams.
- •
Surgical decompression of median nerve resulted in complete resolution.
