Une xe9;tude longitudinale de type cohorte de janvier 2004 xe0; dxe9;cembre 2007 a xe9;txe9; rxe9;alisxe9;e sur 322 enfants suivant la PTME pour recueillir les caractxe9;ristiques de ces enfants et de leurs parents.
Les mxe8;res avaient un xe2;ge moyen de 28,5 ans. Le statut sxe9;rologique des pxe8;res xe9;tait inconnu dans 51,5 % , les couples xe9;taient sxe9;rodiffxe9;rents dans 10,9 % . La prophylaxie xe0; la nxe9;virapine a xe9;txe9; administrxe9;e chez 96,3 % mxe8;res et chez 99,4 % nouveau-nxe9;s ; la rupture prxe9;maturxe9;e des membranes a constituxe9; le principal facteur de risque de TME (34,6 % ) chez les mxe8;res. L’allaitement maternel exclusif avec ablactation xe0; quatre mois a xe9;txe9; rxe9;alisxe9; chez 41 % des enfants. Seuls 4,7 % d’allaitement artificiel ont xe9;txe9; menxe9;s xe0; terme sur les 58,7 % optxe9;s par les mxe8;res xe0; la naissance. Les infections respiratoires aiguxeb;s (38,5 % ), le paludisme (36,9 % ) et la candidose buccale (12,4 % ) ont xe9;txe9; les principales pathologies observxe9;es au cours du suivi. La TME du VIH1 xe0; 18 mois a xe9;txe9; de 9 % (14,58 % sous allaitement maternel ; 5,85 % sous substituts lactxe9;s ; p < 0,03).
L’efficacitxe9; de la stratxe9;gie nationale de PTME repose sur le bon suivi de son protocole. Le passage cependant xe0; une triprophylaxie, la sxe9;curisation de l’alimentation du nourrisson et le diagnostic prxe9;coce permettront de mieux rxe9;duire cette TME du VIH/sida.
This study assesses the management of the prevention of mother-to-child transmission of HIV/AIDS (PMTCT) at Bxe8; Hospital of Lomxe9;, national reference centre in PMTCT.
A longitudinal study, cohort type, was conducted from January 2004 to December 2007 on 322 children following the PMTCT to collect the characteristics of these children and their parents.
The average mothers’ age was 28.5. Fathers’ serological status was unknown in 51.5 % , serodifferents in 10.9 % couples. The nevirapine prophylaxis was administered to 96.3 % mothers and 99,4 % new-borne babies; the early rupture of membranes was the primary risk factor for MTCT (34.6 % ) among mothers. Exclusive breastfeeding with ablactation for 4 months has been achieved in 41 % of children. Only 4.7 % of the artificial feeding have been completed on the 58.7 % mothers who opted for it at birth. Acute respiratory infections (38.5 % ), malaria (36.9 % ) and oral candidiasis (12.4 % ) have been the major diseases observed in the follow-up. MTCT of HIV1 at 18 months old was 9 % (14.58 % for breastfed children, 5.85 % for children fed artificially, P < 0.03).
The effectiveness of the national strategy of PMTCT is based on the observance of its protocol. Crossing however to triprophylaxis, early diagnosis and infant food safety will help reduce this MTCT of HIV/AIDS.
