Pregnancy and breastfeeding represent a challenge to the calcium and bone metabolism of the mother because an adequate amount of calcium needs to be transferred to the fetus and the newborn child for mineralization of the skeleton. Therefore, a moderate calcium and vitamin D supplementation is recommended considering the often insufficient calcium and vitamin D intake via the regular standard Western diet. Back pain which occurs during pregnancy and breastfeeding periods should be clarified as early as possible after the delivery of the child by a diagnostic work-up including imaging techniques in order to allow an informed discussion with the mother on what further therapeutic actions in addition to a continued calcium and vitamin D supplementation should be initiated including a premature termination of breastfeeding. Bisphosphonates are normally prescribed in cases of pregnancy-associated osteoporosis although this is not an evidence-based recommendation because no randomized controlled studies have been carried out for this patient group.
