Ritual cutting of the external female genitalia (FGC), or female genital mutilation (FGM), is practised in many regions of the world and is culturally deeply embedded. This ritual leaves behind massive form and functional impairments of the urogenital region with subsequent impairments in micturition and menstruation as well as disorders of sexual function and parturition. Reconstruction of the female genitalia therefore represents a curative and medically indicated interventional measure. The content depends on the radicality of the FGM, types I-IV according to the World Health Organization (WHO) and simultaneously requires special operative techniques in order to achieve an anatomically functional result. The salvage of the clitoris stump, microsurgical reconstruction of the glans of the clitoris, preputial embedding of the neoglans with an omega domed flap (OD flap) and the complex vulvar reconstruction with an anterior obturator artery perforator flap (aOAP flap) lead to a normalization in form and function of the external female genitalia. For affected girls and women these special operative techniques therefore provide an effective contribution to overcome FGM. Keywords Vulva Clitoris Functional impairment Urogenital region Operation
