Modern medicine provides good treatment options for transgender individuals. Due to the liberal attitudes of society trangender people can nowadays be more self-confident and can openly declare their situation more easily and earlier. Increasingly more countries waive the demand for sterilization within the framework of correction of a gender entry in the register of persons. Subsequently, there are more people with a desire for children who are legally of male gender but have female genital organs and vice versa there are more people who are legally of female gender but have male genital organs: a situation which from the perspective of reproductive medicine cannot simply be ignored. For maintaining fertility a comprehensive counselling of those affected is important before medical gender reassignment in order to explain the individual options for fertility treatment; however, the sometimes restrictive reproductive laws of European countries limit the fulfilment of the desire of transexuals to have children. In the future the legislation must find constructive solutions for this problem in order to put a stop to fertility tourism in other countries often with inadequate control.
