基于住院医师视角的湖北省住院医师规范化培训现状调查

摘要
目的:了解湖北省住院医师规范化培训现状,发现存在的问题并提供建议。方法:自行设计住院医师规范化培训现状调查问卷,对湖北省3市7家住培基地249位住院医师进行调查。结果:83.5%的住院医师认为培训对其有帮助,培训后的能力提升主要体现在临床思维(81.9%)、临床实践(80.3%)、操作技能(69.1%)、人际沟通(66.3%)等方面; 48.6%的住院医师认为病例讨论频率较低或一般,78.2%的住院医师认为带教老师水平较高,50.6%的住院医师表示上级医师平均每周带教次数为2次或以下。回归分析结果显示,年龄、学历、病例讨论频率、带教水平、带教次数是培训认可度的影响因素(P<0.05)。结论:住院医师规范化培训认可度有待提高,且不同特征学员之间存在差异;培训后的效果以临床能力提升为主,职业素养、科研能力、团队合作能力等提升效果不明显;带教情况需要优化。
        Objective: Analyze the situation of training form and ability improvement of the standardized training program for resident physicians in Hubei Province to find the problems and give some suggestions. Methods: A total of 249 residents in 7 hospitals in the 3 cities of Hubei province were selected for a questionnaire survey. Results: 83.5% of the residents considered that the training was helpful. The capacities which were improved most were clinical thinking(81.9%),clinical practice(80.3%),clinical skills(69.1%) and communication(66.3%). 48.6% considered that the frequency of the case discussion was not enough. 78.2%considered that the teachers' ability was of high level. 50.6% considered that the number of the teaching was less than twice per week. Age,education background,the frequency of the case discussion,the teachers' ability and the number of the teaching were the factors of the cognition(P<0.05). Conclusion: The cognition of the standardized training program should be improved. The resident physicians of different characteristics had different cognition of the training. The clinical ability was improved,but there was little improvement in professional ethics,research capacity and team spirit. The teaching needed to be optimized.
