摘要

目的:系统性评价α-硫辛酸联合甲钴胺治疗糖尿病周围神经病变的有效性及安全性。方法:在中英文数据库中检索α-硫辛酸联合甲钴胺治疗糖尿病周围神经病变的随机对照试验(对照组患者单独使用甲钴胺治疗,观察组患者采用α-硫辛酸与甲钴胺联合治疗),按照纳入及排除标准筛选文献后,运用Jada量表评价文献质量,应用RevMan 5.3统计软件进行文献荟萃分析(Meta分析)。结果:共纳入18篇文献,涉及1 728例患者。Meta分析结果显示,观察组患者的临床总有效率明显高于对照组,差异有统计学意义(OR=5.88,95%CI=4.12～8.41,P<0.000 01);治疗后,观察组患者腓总神经运动神经传导速度(MD=5.75,95%CI=3.47～8.03,P<0.000 01)、腓浅神经感觉神经传导速度(MD=4.87,95%CI=0.45～9.82,P=0.03<0.05)明显快于对照组,差异均有统计学意义;两组患者在用药过程中均未出现严重药品不良反应。结论:α-硫辛酸联合甲钴胺治疗糖尿病周围神经病变的临床疗效优于单独使用甲钴胺,且安全性好。
        OBJECTIVE: To systematically evaluate the efficacy and safety of α-lipoic acid combined with mecobalamine in treatment of diabetic peripheral neuropathy. METHODS: Randomized controlled trials(the control group was treated with mecobalamin alone, and the observation group received α-lipoic acid combined with mecobalamin) on the treatment of diabetic peripheral neuropathy with α-lipoic acid combined with mecobalamine were searched in Chinese and English databases. After screening the literature according to the inclusion and exclusion criteria, the Jada scale was used to evaluate the quality of the literature, and RevMan 5.3 statistical software was used for meta-analysis of the literature(Meta analysis). RESULTS: Totally 18 randomized controlled trials were enrolled, including 1 728 patients. Meta-analysis results showed that the total clinical effective rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant(OR=5.88,95%CI=4.12-8.41,P<0.000 01). After treatment, the motor nerve conduction velocity of common peroneal nerve(MD=5.75,95%CI=3.47-8.03,P<0.000 01) and the sensory nerve conduction velocity of superficial peroneal nerve(MD=4.87,95%CI=0.45-9.82,P=0.03<0.05) in the observation group were significantly faster than those in the control group, with statistically significant differences. No severe adverse drug reactions were found in both groups. CONCLUSIONS: The clinical efficacy of α-llipoic acid combined with mecobalamine in the treatment of diabetic peripheral neuropathy is better than that of mecobalamine alone, and the safety is good.
