卡孕栓对人流患者出血量和子宫复旧的影响

摘要
目的探讨卡孕栓对人流患者出血量和子宫复旧的影响。方法将实施无痛人流的引产妇分为两组,根据其意愿分为使用卡孕栓组(观察组)与未使用卡孕栓组(对照组),评估两组术前宫颈松弛情况,记录两组手术时间、术中出血量,及术后阴道流血时间、阴道流血量,术后7d、14d评估子宫复旧情况,统计不良反应及再清宫情况。结果观察组术前宫颈松弛总有效率89.8%,显著高于对照组30.3%(P<0.05);观察组手术时间、术中出血量、术后持续流血时间均显著低于对照组(P<0.05);观察组出血量少于月经量构成比95.6%,显著高于对照组87.6%(P<0.05);观察组月经恢复时间显著短于对照组,术后7d、术后14d子宫内膜厚度均显著高于对照组(P<0.05);术后两组均出现恶心、下腹痛等症状,未经特殊处理,均自行好转,观察组总发生率22.8%,显著低于对照组37.1%(P<0.05),再清宫率比较无统计学意义(P>0.05)。结论卡孕栓能有效促进子宫复旧,减少术后出血,降低手术风险,临床可推广使用。
Objective To investigate the effect of methyl carboprost suppositories on bleeding volume and uterine involution of patients undergoing induced abortion.Methods Lying-in women undergoing painless induced abortion were divided into the observation group(using methyl carboprost suppositories) and the control group(not using methyl carboprost suppositories) according to their intention.The situation of preoperative cervical relaxation in the two groups was evaluated.The operative time,intraoperative blood loss, postoperative vaginal bleeding time and vaginal bleeding volume of the two groups were recorded. The situation of uterine involution was evaluated at 7 d and 14 d after operation. Adverse reactions and re-curettage were statistically analyzed.Results The total effective rate in the observation group was significantly higher than that in the control group(89.8% vs 30.3%)(P<0.05).The operative time, intraoperative blood loss and postoperative bleeding time of the observation group were significantly shorter/less than those of the control group(P<0.05).The constituent ratio of bleeding volume less than menstrual blood volume in the observation group was significantly higher than that in the control group(95.6% vs 87.6%)(P<0.05). The menstrual recovery time of the observation group was significantly shorter than that of the control group,and the endometrial thickness at 7 d and 14 d after operation was significantly larger than that of the control group(P<0.05). There were nausea and hypogastric pain in the two groups but they were relieved without special treatment.The total incidence in the observation group was significantly lower than that in the control group(22.8% vs 37.1%)(P<0.05), but there was no significant difference in the rate of curettage(P>0.05).Conclusion Methyl carboprost suppositories can effectively promote uterine involution, reduce postoperative bleeding and the risk of operation.
