阻塞性睡眠呼吸暂停综合征患者HP感染与血清同型半胱氨酸水平的关系研究

摘要
目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)患者幽门螺旋杆菌(HP)感染与血清同型半胱氨酸(Hcy)水平的关系。方法选取2014年1月至2016年12月德阳市人民医院住院的OSAS患者84例为观察组,同期入院体检健康者作50例作为对照组。比较两组睡眠呼吸和生化检测指标,分析HP感染与OSAS患者关系,Hcy水平与OSAS患者关系,OSAS患者HP感染与Hcy水平的关系。结果观察组睡眠呼吸暂停低通气指数(AHI)、氧减饱和指数(ODI)和血清Hcy水平均高于对照组(P<0.05),观察组夜间最低血氧饱和度(LSpO2)和平均血氧饱和度(MSpO2)低于对照组(P<0.05);观察组中HP阳性患者64例,阴性患者20例,与HP阴性患者比较,HP阳性患者AHI、ODI较高,LSpO2、MSpO2较低(P<0.05);观察组中Hcy水平增高患者55例,Hcy水平降低患者29例,与Hcy水平降低患者比较,Hcy水平增高患者AHI、ODI较高,LSpO2、MSpO2较低(P<0.05);进行相关性分析,HP感染与Hcy水平呈正相关(r=0.354,P<0.05)。结论 Hp阳性者或血清Hcy水平升高的OSAS患者,病情较为严重,OSAS患者中Hp感染与血清Hcy水平呈正相关。
Objective To investigate the relationship between Helicobacter pylori(HP)infection and serum homocysteine(Hcy)levels in patients with obstructive sleep apnea syndrome(OSAS).Methods The total of 84 cases of patients with OSAS who were hospitalized in Deyang People′s Hospital from January 2014 to December 2016 were selected as the observation group,and 50 cases of healthy individuals were enrolled as the control group during the same period.The sleep breath and biochemical indicators were compared between the two groups,and the relationships between HP infection and OSAS,between Hcy concentrations and OSAS,and between HP infection and Hcy concentrations in patients with OSAS were analyzed.Results The sleep apnea hypopnea index(AHI),oxygen desaturation index(ODI)and serum Hcy concentrations in the observation group were significantly higher than those in the control group(P<0.05).The minimum oxygen saturation(LSpO2)and mean oxygen saturation(MSpO2)of the observation group were significantly lower than those of the control group at night(P<0.05).In the observation group,HP was positive in 64 cases and negative in 20 cases,and AHI and ODI were significantly higher while LSpO2 and MSpO2 were significantly lower in HP positive patients than in HP negative patients(P<0.05).In the observation group,Hcy concentrations increased in 55 cases and decreased in 29 cases,and AHI and ODI were significantly higher while LSpO2 and MSpO2 were significantly lower in patients with increased Hcy concentrations than in patients with decreased Hcy concentrations(P<0.05).Correlation analysis showed that HP infection was positively correlated with Hcy concentration(r=0.354,P<0.05).Conclusion HP positive patients with OSAS or patients with elevated serum Hcy concentrations are seriously ill.HP infection is positively correlated with serum Hcy concentrations in patients with OSAS.
