王坤根老师“从肝论治不寐”经验举隅

摘要
[目的]总结第四批全国老中医药专家学术经验继承工作指导老师王坤根教授从肝论治不寐症的诊治思路和临床经验。[方法]随师侍诊,通过对王坤根教授治疗不寐症病案的记录分析,典型医案的收集整理,以及临证思路、辨证规律的探究,总结此病的病因病机及治疗思路。[结果]王坤根教授认为不寐的病机为气血不和,而五脏中调和气血者为肝脏,故王师治疗不寐多从肝入手。肝主疏泄,若疏泄失司,则肝气郁结,郁久化火,易灼伤肝阴,肝阳上亢而致不寐。具体临床分型可分为肝气郁结、肝郁化火、肝火炼痰和肝血亏虚四证,并分别以疏肝理气、清肝泻火、清肝化痰和柔肝养血四法治之,治疗效果斐然。[结论]王师从肝论治不寐,临床特色突出,效果明显,其经验值得同道借鉴。
[Objective] To summarize the fourth batch of the old Chinese medicine experts in the academic experience of the work of the guidance of the teacher Professor WANG Kungen from the liver treatment of insomnia diagnosis and treatment ideas to better inheritance.[Method] With the teacher paternity,Professor WANG Kungen treatment of insomnia disease records for the analysis,the typical medical case collection,finishing,and clinical thinking,dialectical law to explore and summarize the etiology and pathogenesis of the disease and treatment ideas.[Results] Teacher WANG stresses that the main cause of insomnia due to liver,the specific clinical classification can be divided into liver qi stagnation,liver fulminated fire,liver and phlegm and liver blood deficiency four types,and have Shugan qi,Qinggan Xiehuo,liver and phlegm and Rougan nourishing four rules of treatment,the treatment effect striking.[Conclusion] WANG's from the liver treatment insomnia,clinical features prominent,the effect is obvious,his experience worthy of fellows reference.
