CT及MRI在黄色肉芽肿性胆囊炎诊断中的应用

摘要
目的探讨黄色肉芽肿性胆囊炎的CT以及MRI影像学特点,以提高该病的影像学诊断率。方法回顾性分析2010年3月至2017年8月辽宁省铁岭市中心医院收治的12例黄色肉芽肿性胆囊炎患者的影像学资料,以病理学检查结果为标准,分析CT与MRI影像学表现。结果 12例患者均出现胆囊壁不同程度的增厚及胆囊结石,5例增强扫描出现"夹心饼干"征,10例患者胆囊黏膜线完整,2例中断; 1例患者影响到肝脏。结论影像学增强检查的"夹心饼干"征,胆囊壁低密度结节,黏膜线完整,对黄色肉芽肿性胆囊炎的诊断有重要价值。
Objective To explore the CT and MRI features of xanthogranulomatous cholecystitis in order to raise the imaging diagnosis rate of xanthogranulomatous cholecystitis. Methods The imaging data of 12 patients with xanthogranulomatous cholecystitis admitted to Tieling Central Hospital from March 2010 to August 2017 were retrospectively analyzed. Results In all 12 patients,thickening of gallbladder wall and gallbladder stones were observed with varying degrees. In 5 cases, " sandwich biscuit stem" was observed on enhanced scanning in 10 cases,gallbladder mucosal line was intact; in 2 cases,gallbladder mucosal line was interrupted. The liver had been affected in 1 patient. Conclusion Imaging enhanced examination of " sandwich biscuit stem",low density nodules of gallbladder wall and complete mucosal line are of great importance in the diagnosis of xanthogranulomatous cholecystitis.
