超声BI-RADS评分系统联合超声造影用于乳腺肿块良恶性鉴别的诊断价值

摘要
目的分析超声影像学报告及数据系统(BI-RADS)评分系统联合超声造影用于乳腺肿块良恶性鉴别的诊断价值。方法选取60例乳腺肿块患者,对其中的60个肿块均实施常规超声检查,根据BI-RADS评分系统对其实施分级诊断,并展开超声造影检查,比较BI-RADS评分、超声造影单独应用及联合应用时的诊断结果。结果 60个肿块经病理检查,诊断为恶性28个,良性32个。BI-RADS评分单独应用时的敏感度、特异度、准确度为67.86%、75.00%、71.67%,超声造影单独应用时的敏感度、特异度、准确度为71.43%、71.88%、71.67%,BI-RADS评分联合超声造影诊断的敏感度、特异度、准确度为92.86%、93.75%、93.33%。经比较,BI-RADS评分联合超声造影诊断的敏感度、特异度、准确度明显高于两种方法单用时的对应结果 (P<0.05)。结论 BI-RADS评分联合超声造影用于乳腺肿块良恶性鉴别的诊断结果准确度高,可为治疗方案制定、预后评估提供重要参考信息。
        Objective To analyze the diagnostic value of ultrasound imaging report and data system(BI-RADS)scoring system in differentiating benign and malignant breast masses.Methods 60 cases of breast masses were examined by conventional ultrasound.The patients were classified and diagnosed according to BI-RADS scoring system.Contrast-enhanced ultrasonography was carried out to compare the BI-RADS scores.The diagnostic results of angiography alone and in combination.Results After pathological examination,28 of the 60 masses were malignant and 32 were benign.The sensitivity,specificity and accuracy of bi-rads score when applied alone were 67.86%,75.00% and 71.67%;the sensitivity,specificity and accuracy of contrast-enhanced ultrasound score when applied alone were71.43%,71.88% and 71.67%;and the sensitivity,specificity and accuracy of bi-rads score combined with contrastenhanced ultrasound diagnosis were 92.86%,93.75% and 93.33%.After comparison,the sensitivity,specificity and accuracy of bi-rads score combined with contrast-enhanced ultrasound diagnosis were significantly higher than the corresponding results when the two methods were used alone(P<0.05).Conclusions Bi-rads score combined with contrast-enhanced ultrasonography is highly accurate in the diagnosis of benign and malignant breast masses,which can provide important reference information for the formulation of treatment plans and prognosis evaluation.
