四级质量控制管理对腹部手术患者麻醉效果的影响

摘要
目的探讨四级质量控制管理对腹部手术患者麻醉效果的影响。方法在麻醉科主任及专科护士长组织下建立"麻醉科主任-护士长-质控组长-质控小组"四级护理质量管理体系,并应用在腹部手术全麻患者中,比较四级质量控制管理实施前(2017年7—12月)和实施后(2018年1—6月)患者麻醉各项情况。结果四级质量控制管理实施后麻醉躁动发生率低于实施前(P <0. 05),麻醉躁动评分、术后疼痛评分(VAS)低于实施前(P <0. 05),麻醉满意度评分高于实施前(P <0. 05),拔管时间、定向力恢复时间均短于实施前(P <0. 05)。结论四级质量控制管理能有效改善腹部手术患者各项麻醉情况,值得临床推广应用。
Objective To investigate the effect of four-level quality control management on anesthesia of patients undergoing abdominal surgery.Methods To establish the fourth-level nursing quality management system which including " director of anesthesiology,chief nurse,leader of quality control team,and quality control team" under the organization of the director of anesthesiology and the specialist chief nurses. This system was applied to patients undergoing abdominal surgery with general anesthesia. We compared the patient's anesthesia conditions before the implementation of fourth-level nursing quality management system( from July 2017 to December 2017) and after the implementation of the system( from January 2018 to June 2018). Results After the implementation of the four-level quality control management,the incidence of operative agitation after anesthesia was lower than that before the implementation( P < 0. 05). The score of operative agitation after anesthesia and postoperative pain score( VAS) were lower than that before the implementation( P < 0. 05). The anesthesia satisfaction score was higher than that before the implementation( P < 0. 05). And the extubation time and the orientation recovery time were shorter than that before implementation( P < 0. 05). Conclusion Four-level quality control management can improve the anesthesia conditions of patients undergoing abdominal surgery effectively,which was worthy of clinical promotion and application.
