分级诊疗制度下城市二级区医院服务能力现状调查与分析

摘要
目的:掌握城市二级区医院的服务能力现状,为进一步明确如何发挥城市二级医院在分级诊疗体系中的作用提供依据。方法:现场调查国内东部某省城市二级区医院的基本情况及服务能力。结果:城市二级区医院医生和护士数量相对于实际开放床位数不足,高级职称医生人数偏少,高层次学历护理人员短缺;单病种医疗服务开展情况欠佳;一级、二级必设诊疗科室设置不全;三级学科能够诊治的常见病种、疑难病种以及开展的关键诊疗技术或手术数量有限。结论:二级区医院现有临床服务能力限制了其在分级诊疗体系中发挥作用;各地市应结合本地区城市二级区医院实际情况和区位特点,通过建立多种形式的医联体,因地制宜地探索如何发挥二级区医院在分级诊疗体系中的功能。
Objectives: To understand the situation of the service capacity of the secondary district hospitals in the cities and to provide evidence for how to play the role of the secondary hospitals under the hierarchical medical system. Methods: The main status and the service capacity of the secondary district hospitals in the cities of the province in eastern China were investigated. Results: The number of doctors and nurses in the secondary district hospitals were insufficient related to the number of the actual open beds. The number of doctors with senior professional title and the nurses who were highly educated were insufficient. The first-level and the second-level obligatory facilities were set up incompletely. There was limited number of the common diseases and the difficult diseases diagnosed and treated by the third-level obligatory facilities,and the key therapeutic techniques or the surgeries. Conclusions: The secondary hospital played poor role in hierarchical medical system due to the limited capacity. It is considered to construct various forms of medical association matched the situations of secondary hospital capacity and healthcare system in the city to full play its role under hierarchical medical system.
