上海市医疗服务价格调整后公立医院医疗收支构成变动分析

摘要
目的:通过比较分析2014—2017年几轮上海市医疗服务价格调整政策实施后公立医院医疗收支结构转变情况,为医疗服务价格改革成效评价提供参考,也为今后医疗服务价格政策调整完善提供依据。方法:横向与纵向统计性描述对比分析。结果与结论:上海市公立医院取消药品加成已取得显著成效,同时医务性收入结构总体趋好,但尚存在药占比仍未达标、卫生材料费与检查收入过高等问题。为此,建议应提升产业集中度,加强医院成本管控,并逐步建立新型医疗服务价格动态调整机制。
Objective: Through comparative analysis of the structural transformation of medical income and expenditure after the implementation of medical services price adjustment policy in Shanghai from 2014 to 2017, so as to provide references for evaluation of effectiveness of medical service price reform, and to provide basis for future adjustment and improvement of medical service price policy. Methods: Comparative analysis of horizontal and longitudinal statistical description were applied. Results and Conclusion:Shanghai public hospitals have achieved remarkable results in abolishing markups on pharmaceuticals, and medical income structure was generally getting better. However, there were still some problems, such that the drug proportion was still not up to standard, the health materials fees and inspection incomes were too high, etc. Therefore, it was suggested to promote the industrial concentration,strengthen hospital cost and gradually establish a new dynamic adjustment mechanism for medical service prices.
