摘要

目的探讨心肌酶谱、高敏肌钙蛋白(hs-cTnl)在小儿手足口并发心肌炎的诊断意义。方法将我院2016年1月～2018年6月收治的124例小儿手足口病作为研究对象,其中62例为并发心肌炎患儿,62例为未并发心肌炎患儿。对患者均进行心肌酶谱、高敏肌钙蛋白检测。比较两组患儿心肌酶谱、高敏肌钙蛋白水平间的差异与异常率。结果手足口病并发心肌炎组患者hs-cTnl[(426.9±32.6)pg/mL]、CK[(321.5±32.9)U/L]、AST[(65.2±7.3)U/L]、LDH[(187.3±43.8)U/L]水平显著高于手足口病未并发心肌炎hs-c Tnl[(89.6±23.8)pg/m L]、CK[(123.7±15.0)U/L]、AST[(21.6±3.9)U/L]、LDH[(90.6±21.9)U/L],比较差异有统计学意义(P <0.05);手足口病并发心肌炎组在hs-cTnl(67.74%)、CK(62.90%)、AST(64.52%)、LDH(66.13%)异常率上均比手足口病未并发心肌炎组高(P <0.05)。结论心肌酶谱、高敏肌钙蛋白在小儿手足口并发心肌炎具有较高的诊断价值,有利于指导疾病诊断及治疗,值得临床选择。
        Objective To explore the diagnostic significance of myocardial enzymes and high-sensitivity troponin(hscTn1)in children with hand-foot-and-mouth disease complicated with myocarditis. Methods One hundred and twentyfour children with hand-foot-and-mouth disease who were admitted and treated in our hospital from January 2016 to June 2018 were selected as research objects.Among whom,there were 62 children complicated with myocarditis and 62 children without complication of myocarditis.Myocardial enzymes and high-sensitivity troponin of all children were tested.The differences and abnormality rate of myocardial enzyme and hs-cTn1 level of children were compared between two groups.Results The hs-cTn1[(426.9±32.6)pg/mL],CK[(321.5±32.9)U/L],AST[(65.2±7.3)U/L]and LDH[(187.3±43.8)U/L]levels of children with hand-foot-and-mouth disease complicated with myocarditis were significantly higher than those of children with hand-foot-and-mouth disease without complication of myocarditis[(89.6±23.8)pg/mL],[(123.7±15.0)U/L],[(21.6±3.9)U/L]and[(90.6±21.9)U/L].The comparative differences were statistically significant(P < 0.05).The abnormality rates of hs-cTn1,CK,AST and LDH in children with hand-foot-and-mouth disease complicated with myocarditis were67.74%,62.90%,64.52% and 66.13% respectively,significantly higher than those of children with hand-foot-and-mouth disease without complication of myocarditis(P < 0.05). Conclusion Myocardial enzymes and high-sensitivity troponin has higher diagnostic values in hand-foot-and-mouth disease complicated with myocarditis,which is helpful to guide the diagnosis and treatment of diseases.It is worthy of clinical application.
