摘要
目的采用静息态功能磁共振成像(f MRI)、扩散张量成像(DTI)技术观察精神分裂症患者小脑功能连接与解剖连接的特点。方法选取2015年1月至2017年1月于我院接受f MRI、DTI检查的25例精神分裂症患者作为观察组,另选取同期体检健康的我院职工10例作为对照组。所有研究对象均行静息态脑f MRI以及DTI检查,通过后处理技术获取功能连接相关系数r、小脑中脚平均FA值,并通过统计学方法分析精神分裂症患者小脑功能连接、解剖连接与正常健康人的差异。结果观察组与对照组比较Seed1正相关降低区域、负相关降低区域分别为双侧舌回、右侧额中回,而与Seed2正相关降低区域则为双侧舌回、左侧中央前回,而Seed2负相关降低的区域则为左侧缘上回;观察组患者的左侧小脑中脚FA值明显低于对照组受检者(P<0.05);但两组受检者右侧的小脑中脚FA值比较则无显著差异(P>0.05);精神分裂症患者右侧小脑-右侧舌回连接强度与右侧小脑中脚FA值具有显著的正性相关关系,r=0.851,P<0.05,具有统计学意义。结论采用f MRI、DTI联合的MRI检查方案能够发现静息状态下精神分裂症患者小脑与大脑皮层某区域连接存在降低以及小脑中脚局部白质纤维完整性受损的表现,这一表现或二者之间的相关性可提示功能连接与解剖连接之间具有密切的联系。
Objective To observe the effect of functional connectivity and anatomical connectivity of the cerebellum in patients with schizophrenia application resting state functional magnetic resonance imaging(f MRI)and diffusion tensor imaging(DTI).Methods Choose from January 2015 to January 2017 in our hospital to accept f MRI,DTI examination of 25 patients with schizophrenia as observation group,Another 10 healthy workers in our hospital were selected as control group.All subjects were examined by resting state brain f MRI and DTI,the correlation coefficient R and the average FA value of the middle cerebellar peduncle were obtained by post-processing,Statistical analysis was used to analyze the differences of functional connectivity and anatomical connectivity of the patients with schizophrenia.Results Compared with the control group,the positive correlation of Seed1 was decreased in the observation group,The area of negative correlation was bilateral lingual gyrus and right middle frontal gyrus,while the area of positive correlation with Seed2 was bilateral lingual gyrus and left precentral gyrus,The negative correlation of Seed2 was decreased in the left superior frontal gyrus;In the observation group,the FA value of the left middle cerebellar peduncle was significantly lower than that of the control group(P<0.05);However,there was no significant difference in FA value between the two groups(P>0.05);There was a significant positive correlation between the right cerebellar right lingual gyrus and the FA value of the right middle cerebellar peduncle,R=0.851,P<0.05,with statistical significance.Conclusions By MRI f MRI,DTI of the joint inspection plan can be found in the resting state in schizophrenic patients with cerebellar cortex area is reduced and the performance of connection exists medipeduncle damaged local white matter fiber integrity,the correlation between the performance or two can be connected with the anatomical prompt function is related to the connection between.
