摘要
目的:探讨足外侧柱延长术联合趾长屈肌腱(flexor digitorum,longus,FDL)转位术后辅以中医康复治疗可复性平足症的临床疗效观察。方法:我院采用足外侧柱延长术联合FDL转位术治疗可复性平足症患者30例,男17例,女13例;年龄15-43岁,平均年龄26.7岁;左足16例,右足14例。术后辅以中医康复治疗,采用美国足与踝关节协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与足评分标准对其治疗前后临床疗效进行评估。结果:所有患者术后随访10-36个月,平均21.3个月。末次随访AOFAS踝与足评分为70-100分,其中优13例,良15例,可2例,优良率为93.3%(28/30)。所有患者足弓高度及前足外展、后足外翻畸形均取得较好恢复,可正常行走,术后X线片测量可复性平足症特征角度明显改善(P<0.01)。结论:足外侧柱延长术联合FDI转位术后辅以中医康复治疗可复性平足症临床疗效显著。
Objective:To discuss The rehabilitation therapy of traditional Chinese medicine of recoverability flatfoot with lateral column lengthening combined flexor digitorum longus transposition.Methods:30 cases of recoverability flatfoot were treated by lateral column lengthening combined FDL transposition.There were 17 males and 13 females with an mean age of 26.7 years(rangl5 to 43 years).The left foot was involved in 16 cases and the right was affected inl4 cases,with the rehabilitation therapy of traditional Chinese medicine after surgery.American Orthopaedic Foot and Ankle Society(AOFAS) score was used to evaluate functions of ankle and foot before and after operation.Result:Eventually,all the patients were followed up with an average duration of 21.3 months(range 10-36months).AOFAS scores were 90-100 points in 13 patients,80-90 points in 15 patients and the excellent and good rate was 93.3%(28/30).The specific indexes of X-ray improved distinctly,and the difference had statistically significances(P <0.01).The arch height and foot outreach and sufficient evaginate of all the patients had achieved good recovery,could walk normally.Conclusions;The lateral column lengthening combined FDL transposition for recoverability flatfoot should be combined with the rehabilitation therapy of traditional Chinese medicine.The clinical outcome is significant.
