Yellow and white tongue coating color may reflect contrary inflammatory status via serum interleukin-17α level

Tongue coating is an important diagnostic indicator in traditional Chinese medicine(TCM). A typical white or yellow tongue indicate external/Cold ZHENG and internal/Heat ZHENG, respectively, which are closely related to different inflammatory stimuli that lead to the development of digestive system diseases. The aim of this study was to test the hypothesis that coating color is dictated by inflammatory cytokines. A hospital-based case-control study included 142 patients with gastric cancer(GC) and 98 healthy controls, whose coating color and texture of each individual were recorded. An electrochemiluminescence assay was used to measure serum levels of 20 inflammatory cytokines. The results showed that a yellow tongue coating correlated with higher serum interleukin-17α(IL-17α) levels in both GC patients(Z=2.612, P=0.009) and controls(Z=2.279, P=0.023), with receiver operating characteristics(ROC) curve cutoff values of 6.033 pg/ml and 3.507 pg/ml, respectively. Correlation network analysis showed that there were identical 11 pairs of negatively correlated cytokines in the GC patients and controls, respectively. Accordingly, these results suggest that the yellow and white tongue coating may reflect contrary inflammatory status via different serum interleukin-17α levels in both GC patients and controls.
