胃安泰胶囊治疗胃癌前期病变的临床研究

摘要
本论文是山西中医学院第二中医院承担的山西省科委自然基金课题“胃安泰胶囊治疗胃癌前期病变的实验研究及临床观察”中的一部分。在导师和科室老师指导下，研究生完成其部分工作，论文从临床研究方面进行论述。临床应用胃安泰胶囊治疗符合纳入标准的胃癌前期病变患者35例，并与维酶素治疗的23例相对照，疗程3～6个月，观察结果显示：治疗组在改善临床症状，逆转胃癌前期病变等方面均不同程度优于对照组，其中部分指标与对照组存在显著性差异。治疗组的显效率为54.29％，症状改善总有效率为91.42％；对照组的显效率为13.04％，症状改善总有效率为21.74％，两组比较有显著性意义。治疗组对胀满、疼痛、纳呆、嗳气、大便稀溏、面色萎黄、消瘦等主要症状改善效果比西药对照组为优(P＜0.01)。说明胃安泰胶囊可能具有增强胃黏膜的屏障作用；调节胃肠运动，阻止胆汁反流；免疫调节等作用，在中医药对胃癌前期病变的治疗作用中具有较明显的疗效。通过本研究表明：中医药治疗胃癌前期病变的疗效是确切的；脾虚气滞兼胃络瘀阻为胃癌前期病变的主要病理变化；益气健脾，理气消痞，化瘀解毒为治疗胃癌前期病变的有效法则；胃安泰胶囊为治疗胃癌前期病变的有效中药制剂。
The study is a part of the natural fund task of shanxi science and technology committee, which is "The experimental research and clinical observation of Weiantai jiao nang's treatment to precancerous lesions of gastric caner". With the help of tutor and office teachers, the graduate has finished a part of the job. the thesis deals with the clinical research. In the clinic, we use Weiantai jiao nang to treat 35 cases of precancerous lesions of gastric caner. and contrast with 23 cases applied with Wei Mei Su, the result shows that the treat group is superior to the contrast group in the asfects of improving clinical symptoms and reversing precancerous lesions of gastric caner, it has remarkable difference compared with the contrast gro-up. The effective ratio of treat group is 54.29%, the sum effective ratio to the improvement of symptoms is 91.42%; and they are 13.04% and 21.74% of contrast group, there are siginificant differences between the two groups. The treatment group is superior to the contrast group in the effects of improve symptoms including distending, stomachache, anorexia, belching, loose stool, pale complexion, lean, etc. It exposed that Weiantai jiao nang maybe has the function of enhance gastric macasa; adjust the stomach's movement and prevent the reflux of bile gall. The research shows that: the curative effect of the Traditional Chinese Medicine are reliable; the major pathological reason of precancerous lesions of gastric caner is spleen deficiency and stagnation of qi and stasis of blood; the therapeutic principle to precancerous lesions of gastric caner is strengthen the qi and replenish the spleen; regulating the qi and disperse the mass; remove the blood stasis and clear away the toxic; Weiantai jiao nang is an effective drug of Traditional Chinese Medicine in. the treatment of precancerous lesions of gastric caner.
