中医药内外兼治对125例湿热蕴脾型口腔扁平苔藓患者临床疗效观察

摘要
目的:探讨中医药内外兼治对口腔扁平苔藓(OLP)的临床疗效。方法:将125例湿热蕴脾型OLP患者随机分为两组:对照组(50例)和治疗组(75例),分别给予中药外涂治疗,与中药外涂和自拟苔藓颗粒口服治疗。观察两组患者治疗前后临床疗效、口腔病损评分、中医临床证候程度评分变化。结果:治疗组总有效率(94.67%)显著优于对照组(78.00%)(P<0.05);治疗后,两组口腔病损程度评分与治疗前比较均显著降低(P<0.05,P<0.01),且治疗组治疗前后差值显著低于对照组(P<0.01)。同组治疗前后湿热蕴脾临床症状、体征比较,治疗组中失眠多梦较前有显著改善(P<0.05),而其他症状、体征程度较前有极显著改善(P<0.01);对照组中仅疼痛感、粗糙感较前有极显著改善(P<0.01),口干舌燥、口舌生疮、情志不舒较前有显著改善(P<0.05)。结论:中医内外兼治法可明显提高OLP的临床疗效及口腔局部病损、中医临床症状、体征程度。
Objective: To explore and analyze clinical efficacy of external and internal treatment by TCM on oral lichen planus(OLP). Methods: A total of 125 cases of OLP with dampness-heat stagnating in spleen syndrome were randomly divided into two groups, which were the control group(50 cases) and the treatment group(75 cases). Control group was treated with external application of Chinese medicine, treatment group was treated with the Self-made Bryophyte Granules combine with external application of Chinese medicine. The clinical efficacy, oral lesion score, and TCM clinical syndrome score of the two groups were observed between before and after treatment. Results: The total effective rate of the treatment group was 94.67%, significantly better than that of the control group(78.00%)(P<0.05). After the treatment, the level of oral lesions in the two groups reduced than that before the treatment(P<0.05, P<0.01), and the difference befween before and after treatment of the treatment group was better than the control group(P<0.01). In the comparison of TCM clinical syndromes of before and after treatment in the treatment group, most of the symptoms was significantly(P<0.01), while the degree of insomnia and dreamy syndrome was significantly improved(P<0.05). In the control group, the TCM clinical syndromes such as the senses of pain and roughness were significantly better than before treatment(P<0.01). Moreover, syndromes such as sore of tongue, mouth parched and tongue scorched and unpleasant feeling were obvious better than that before treatment(P<0.05). Conclusion: Internal and external treatment of TCM can significantly improve the clinical efficacy, local lesions of OLP, and the degree of TCM clinical syndromes.
