血清白细胞介素22和白细胞介素22受体1在胎膜早破绒毛膜羊膜炎患者中的表达及临床意义

摘要
目的探讨血清白细胞介素22(IL-22)和白细胞介素22受体1(IL-22R1)在胎膜早破绒毛膜羊膜炎患者中的表达及临床意义。方法选取胎膜早破100例作为研究对象,按照绒毛膜羊膜感染程度将其分为无绒毛膜羊膜炎组(26例)、亚临床绒毛膜羊膜炎组(35例)和临床绒毛膜羊膜炎组(39例)3组,另随机选取同期32例正常足月妊娠孕妇作为对照组。观察比较胎膜早破患者与正常足月妊娠孕妇血清IL-22、IL-22R1及白细胞、C反应蛋白(CRP)水平;无绒毛膜羊膜炎组、亚临床绒毛膜羊膜炎组及临床绒毛膜羊膜炎组血清IL-22、IL-22R1及白细胞、CRP水平。结果胎膜早破患者血清IL-22、IL-22R1及白细胞、CRP水平均高于正常足月妊娠孕妇,差异有统计学意义(P<0.05)。无绒毛膜羊膜炎组、亚临床绒毛膜羊膜炎组和临床绒毛膜羊膜炎组血清IL-22、IL-22R1及白细胞、C反应蛋白水平总体比较差异均有统计学意义(P<0.05)。血清IL-22、IL-22R1及白细胞、C反应蛋白水平临床绒毛膜羊膜炎组高于亚临床绒毛膜羊膜炎组和无绒毛膜羊膜炎组,亚临床绒毛膜羊膜炎组高于无绒毛膜羊膜炎组,差异均有统计学意义(P<0.05)。结论血清IL-22和IL-22R1在胎膜早破产妇中高表达,对预测绒毛膜羊膜炎有一定临床意义,为预测亚临床绒毛膜羊膜炎的重要指标。
Objective To analyze the expression and clinical significance of serum interleukin 22(IL-22) and IL-22 R1 in patients with premature rupture of membranes and chorioamnionitis. Methods A total of 100 patients with premature rupture of membranes who were admitted to our hospital were enrolled in the study. According to the degree of infection of chorioamnionitis, the patients were divided into non-chorioamnionitis group(n=26), subclinical chorioamnionitis group(n=35) and clinical chorioamnionitis group(n=39). Another 32 healthy full-term women were randomly selected as the control group during the same period. The levels of IL-22, IL-22 R1, white blood cell(WBC), and C-reaction protein(CRP) were compared between the patients with premature rupture of membranes and those healthy full-term pregnant women. The levels of serum IL-22, IL-22 R1, WBC count and CRP in the non-chorioamnionitis group, subclinical chorioamnionitis group and clinical chorioamnionitis group were observed and compared. Results The serum levels of IL-22, IL-22 R1, WBC and CRP in the group with premature rupture of membranes were significantly higher than healthy full-term women, and the difference was statistically significant(P<0.05). The differences were statistically significant in serum IL-22, IL-22 R1, WBC count and CRP levels between the non-chorioamnionitis group, subclinical chorioamnion group and clinical chorioamnionitis group(P<0.05). The expression levels of serum IL-22, IL-22 R1, WBC count and CRP in the clinical chorioamnionitis group were higher than those in subclinical chorioamnionitis group and non-chorioamnion group, and the levels in subclinical chorioamnionitis group were higher than those in non-chorioamnion group, suggesting significant difference(P<0.05). Conclusion The levels of serum IL-22 and IL-22 R1 are highly expressed in puerpera with premature rupture of membranes, which has certain clinical significance for predicting chorioamnionitis, and were important indices for predicting subclinical chorioamnionitis.
